What Is Chronic Kidney Disease?
Chronic kidney disease (CKD) is the gradual, usually permanent loss of kidney function over months or years. Healthy kidneys filter waste and extra fluid from the blood, balance minerals, and help control blood pressure. As CKD advances, waste builds up in the body. In its final stage — kidney failure, also called end-stage renal disease (ESRD) — a person needs dialysis or a kidney transplant to stay alive.
In India, diabetes and high blood pressure are the two leading causes of CKD. Because early CKD usually has no symptoms, it is often discovered late — which is why anyone with diabetes, hypertension, or a family history of kidney disease should have kidney function (creatinine/eGFR) and urine protein checked regularly.
The Five Stages of Kidney Disease
Doctors grade CKD by eGFR (estimated glomerular filtration rate), a blood-test measure of how well the kidneys filter:
- Stage 1 — kidney damage with normal filtration (eGFR 90+). Usually no symptoms.
- Stage 2 — mild loss of function (eGFR 60–89).
- Stage 3 — moderate loss (eGFR 30–59). Fatigue, swelling, and blood-pressure changes may begin.
- Stage 4 — severe loss (eGFR 15–29). Time to plan for dialysis or transplant with a nephrologist.
- Stage 5 — kidney failure (eGFR below 15). Dialysis or a transplant is needed to live.
Dialysis vs. Kidney Transplant
Both treat kidney failure, but they work very differently. For medically eligible patients, transplant generally offers longer survival and better quality of life — which is why nephrologists often call it the preferred treatment for ESRD.
| Dialysis | Kidney Transplant | |
|---|---|---|
| What it is | A machine (haemodialysis) or the abdominal lining (peritoneal dialysis) filters the blood. | Surgery places a healthy donated kidney that takes over filtration. |
| Time commitment | Typically 3 sessions of ~4 hours weekly (HD), or daily exchanges (PD), ongoing. | One surgery + recovery, then regular follow-up visits and daily medication. |
| Diet & lifestyle | Strict fluid and diet limits. | Far fewer restrictions after recovery. |
| Main requirement | Vascular access / catheter; ongoing centre availability. | A medically matched, lawful donor; fitness for surgery; lifelong immunosuppressants. |

The Lawful Kidney Transplant Process in India
Kidney transplantation in India is governed by the Transplantation of Human Organs and Tissues Act (THOTA), 1994. The law exists to protect both donors and recipients: organ donation must be voluntary and unpaid, and buying or selling an organ is a criminal offence. A typical living-donor transplant proceeds like this:
- Nephrology evaluation — confirming that transplant is the right treatment and the patient is fit for surgery.
- Donor identification — usually a near relative (spouse, parent, sibling, child, grandparent, grandchild). Others require state Authorisation Committee approval.
- Medical workup — blood group and tissue compatibility, kidney function, anaesthesia fitness, and screening to protect the donor's long-term health.
- Legal verification — identity, relationship documentation, and committee review where required, ensuring the donation is voluntary and unpaid.
- Surgery — donor and recipient operations by the transplant team, typically with a hospital stay of about a week for each.
- Lifelong follow-up — immunosuppressant medication and scheduled reviews for the recipient; periodic health checks for the donor.
Patients without a living donor can register for a deceased-donor kidney through NOTTO (the National Organ and Tissue Transplant Organisation) and its state registries. Waiting times vary by state and blood group. Paired (swap) donation is also legal when two donor–recipient pairs are incompatible within themselves but compatible across pairs.
Costs, Insurance & Government Schemes
Transplant costs vary with the clinical situation, so beware of any website quoting a fixed price — or any monetary figure attached to an organ itself, which is a hallmark of illegal brokering. Legitimate cost components are hospital and surgical charges, medication, and follow-up care. Support available in India includes:
- Ayushman Bharat (AB-PMJAY) — covers kidney transplant for eligible families up to the scheme limit.
- State schemes — several states run additional transplant/dialysis support programmes.
- Private & corporate insurance — many policies cover transplant; verify your plan's terms before treatment planning.
Recovery and Life After Transplant
Most recipients leave hospital within one to two weeks and return to normal activities over two to three months. Lifelong immunosuppressant medication prevents rejection, and regular follow-up lets the team adjust doses and catch problems early. Living donors are usually discharged within a week and can expect to return to their routine in four to six weeks, with most living completely normal lives afterwards.

Frequently Asked Questions
Can I sell my kidney in India?
No. Buying or selling human organs is a criminal offence in India under the Transplantation of Human Organs and Tissues Act (THOTA), 1994. Kidney donation is legal only as a voluntary, unpaid gift — typically from a near relative, or with Authorisation Committee approval in other cases.
Who can legally donate a kidney to a patient in India?
Near relatives — spouse, parents, siblings, children, grandparents and grandchildren — can donate after medical evaluation and verification. Donors who are not near relatives require approval from the state Authorisation Committee, which must be satisfied the donation is out of affection and involves no payment.
Can a person live a normal life with one kidney?
Most people who donate a kidney after thorough medical evaluation live full, healthy lives. The remaining kidney increases its capacity to compensate. Donors need lifelong routine follow-up, which the transplant team plans before surgery.
What is the difference between dialysis and a kidney transplant?
Dialysis is an ongoing treatment that filters the blood when the kidneys cannot — typically several sessions every week, for life or until transplant. A transplant replaces the failed kidney with a healthy donated one; for eligible patients it usually offers better long-term survival and quality of life, but it requires surgery and lifelong anti-rejection medication.
How long is the wait for a deceased-donor kidney in India?
Waiting time varies by state, blood group, and registry position under NOTTO (the National Organ and Tissue Transplant Organisation) and its state bodies. It can range from months to several years, which is why medically suitable living donation from a near relative is often discussed first.
Is a kidney transplant covered by insurance or government schemes?
Ayushman Bharat (AB-PMJAY) covers kidney transplant for eligible families up to its policy limit, several state schemes provide additional support, and many private insurance and corporate policies cover transplant. Coverage depends on your specific plan and eligibility — our team can help you verify yours before treatment planning.
What are the early warning signs of kidney disease?
Early chronic kidney disease often has no symptoms. Signs that appear as it progresses include swelling of the feet and ankles, persistent fatigue, foamy urine, changes in urination, poorly controlled blood pressure, and nausea. People with diabetes or high blood pressure should have kidney function tested regularly even without symptoms.
