Your Parents Had Kidney Disease. Here's Your Prevention Plan.

If your parents have kidney disease, genetics loaded the gun. But you control whether it fires. This article gives you a month-by-month prevention plan if one or both parents have CKD, PKD, or FSGS.
Step 1: Know Your Parents' Diagnosis - First, understand what kidney disease your parents have: CKD (Chronic Kidney Disease) - most common, 1 in 3 adults. PKD (Polycystic Kidney Disease) - genetic, 1 in 400 people. FSGS (Focal Segmental Glomerulonephritis) - immune-based, genetic component. IgA Nephropathy - most common glomerulonephritis worldwide. Ask your parents about their diagnosis, stage, medications, treatment status.
Step 2: Get Genetic Counseling - See a genetic counselor, get genetic testing if available, understand your risk percentage (25%, 50%, or 100% depending on disease type), learn what to watch for.
Step 3: Establish Baseline - Get blood creatinine test, eGFR measurement, urine albumin check, blood pressure monitoring, blood glucose fasting, cholesterol panel, kidney ultrasound (if family history of PKD).
Step 4: Optimize Blood Pressure (Month 1) - Target: less than 120/80 mmHg. Actions: Home blood pressure monitoring, reduce salt to less than 2,300 mg/day, exercise 30 mins 5 days/week, lose weight if BMI greater than 25, limit alcohol, manage stress. After 1 month if still elevated: start ACE inhibitor or ARB medication.
Step 5: Diabetes Prevention (Month 2) - Get fasting blood glucose test, get HbA1c test. If pre-diabetic (HbA1c 5.7-6.4%): join diabetes prevention program, lose 5-10% of body weight, exercise 150 mins/week, eat whole grains not refined carbs, reduce sugar drinks.
Step 6: Optimize Cholesterol (Month 3) - Target: LDL less than 100 mg/dL. Actions: Eat heart-healthy diet (Mediterranean style), increase fiber, reduce saturated fat. If LDL not at target: start statin medication.
Step 7: Lifestyle Foundation - Lose 10% body weight (reduces proteinuria 10-20%), reduce salt 50% (lowers BP 5-7 mmHg), exercise 150 mins/week (improves insulin sensitivity), Mediterranean diet (reduces kidney disease progression 30-40%), quit smoking (improves kidney blood flow 20% immediately).
Step 8: Screening Schedule - Low risk: annually, Moderate risk: every 6 months, High risk: quarterly, Very high risk: monthly plus ultrasound yearly.
Step 9: Prevention Medications - ACE inhibitor or ARB, SGLT2 inhibitor (if diabetic or high-risk), Statin, Aspirin (if very high risk).
Step 10: Build Support Network - Find a kidney specialist, join kidney disease support group, get family members tested (siblings have same risk), share results with parents (compliance improves outcomes).
Your parents' kidney disease is information, not a sentence. You have time—often 10-20 years—to prevent or dramatically slow kidney disease progression. Start now. These changes compound. Ten years from now, you'll be glad you didn't wait.
